  • A sense of security, incredible team support, and flexibility for true work-life balanceThe Home Health Patient Care Manager and RN Hybrid is responsible for the supervision and coordination of clinical services and provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The role involves:
    • Receiving referrals and ensuring appropriate clinician assignments
    • Coordinating patient needs assessments and care plans
    • Managing phone calls and new orders from physicians and patients
    • Supervising care provided by home health aides and licensed practical vocational nurses
    • Developing, implementing, and revising care plans in collaboration with physicians and other healthcare professionals
    • Communicating with agency staff, physicians, patients, and family membersResponsibilities include:
      • Accurately completing comprehensive assessments (OASIS) and medication reconciliation
      • Directly or indirectly supervising care provided by home health aides and licensed practical vocational nurses
      • Providing instruction and assigning tasks according to State and federal regulations
      • Initiating, developing, implementing, and revising care plans in collaboration with physicians and other healthcare professionals
      • Communicating relevant information with agency staff, physicians, patients, and family membersRequirements include:
        • Current RN licensure in the state of practice
        • Current CPR certification required
        • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
