We are seeking Board Certified/Board Eligible Geriatric, Family or Adult Medicine Advanced Practice Providers for outpatient geriatric primary care positions in Orlando, Florida (west side).The CenterWell Difference: As a Provider with CenterWell Senior Primary Care, you ll have the opportunity take a hands-on approach to addressing patient needs inside the exam room, as well as looking beyond to understand how social determinates of health are impacting health outcomes.   • Active, unrestricted NP/PA license in the appropriate state
  • 2-5 Years Primary Care Experience with Chronic Disease Management
  • Current certification as an NP or PA
  • Current DEA license or ability to obtain a DEA license
  • This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
